At one time some banks, better to remain unnamed, had a habit of losing papers or not following written instructions regarding my wishes on what should happen to my account/s. It didn't involve a lot but when you don't have much you have to look after every penny and I didn't like thier cavalier attitude and that is being unfair to cavaliers.
I then chose to give my instructions in writing with a little paragrath at the bottom of a copy of the letter which required a signature confirming that the person had received the letter acknowledged same and duly carried out my instructions, together with a self addressed envelope for them to send the copy letter, duly signed, back to me. It was a pain to do but legally I felt it was necessary as these B*&^*&^%%$$ds didn't really care what they did as long as they took thier salary home every month.
I know it is a lot of time and trouble to do this but maybe it is a necessary evil and people should make a habit of asking for such a sensible way of ensuring there is proof of whatever request is/ was made, together with proof of any permissions granted.
All too easy to see this in hindsight but follow this method and I can assure you it works.
